Dr. Neal Mermelstein, DO,MS, BA

Medical Director

Neal Mermelstein Dr. Neal Mermelstein is dually certified in both Family Medicine and Geriatrics. Actively involved in Hospice and Palliative Medicine.

Dr. Mermelstein has been in practice since 1989 and has been the Medical Director at Deer Meadows for over 30 years.This involves overseeing the full spectrum of care rendered to residents including quality of care, rehabilitation medicine, occupational therapy, pharmacy, as well as psychological services. Dr Mermelstein serves as a Board Member for Baptist Home Foundation.

Dr. Mermelstein also maintains a full outpatient medical practice and is affiliated with both Jefferson Healthcare and Nazareth Hospital. Attending physician in both hospitals for over 25 years.

Dr. Mermelstein serves as the Medical Director for Compassionate Care and Regional Medical Director for Bridge Care Hospice. He is well versed in end of life issues to ensure comfort care to both the patient and family.

Dr. Mermelstein also has the privilege of being the Medical Director for Deer Meadows Home Health Services. In this current role he is actively involved in developing protocols to help provide comprehensive programs to keep patients at home by closely monitoring their health, ensure proper compliance with medications and diet. Working closely with nursing staff, rehab and occupational therapist as well as with social services to optimize their care at home.

Dr. Mermelstein has received multiple awards for his distinguished medical services including the Pennsylvania Medical Society.

In his free time he enjoys playing tennis, riding bicycles and taking walks in the park with his wife and four beautiful children.


Sapan Majmundar, DO

Dr. Majmundar was born in Hoboken, NJ and grew up in Monmouth County, NJ. He graduated Magna Cum Laude from State University at New Paltz, where he also played on the Lacrosse Team. He attended medical school at New York College of Osteopathic Medicine, and subsequently completed his residency in Internal Medicine at Hackensack Palisades Medical Center in 2014. Since that time, Dr. Majmundar has cared for patients in a myriad of settings; including outpatient, hospital, nursing homes, LTACs, SNFs, and has served as associate Director of Hospital Medicine at Atlantic Health-Newton Medical Center and is currently Medical Director for Brookdale Hospice. Dr. Majmundar is passionate about teaching residents, nurses and his patients. He has published in multiple medical journals and recently received Bayshore Medical Center's 2019 Physician of the Year award. In his free time, Dr. Majmundar is an avid golfer and loves to travel with his wife and family. Dr. Majmundar is Board Certified in Internal Medicine.


Dr. Luvkarnjit Singh, MD

Attending Physician

Luvkarnjit Singh

Board Certified in Internal Medicine. Graduated from Rutgers University and went on to graduate from the University of Medicine and Dentistry Newark, NJ. He is interested in managing medically complicated advanced aged patients with multiple comorbidities. He is Assistant Medical Director at Attleboro Nursing and Rehabilitation Center, one of the largest subacute nursing facilities in the Bucks County Area. He is also Medical Director of Silver Lake Nursing Home and Associate Director of Compassus Hospice. In his personal time, he likes to spend time with his wife and children. Dr. Singh's hobbies include sports and traveling.


Dr. Kwasi A. Manu, MD

Kwasi A. ManuDr. Kwasi A. Manu, (aka Dr. K), a general surgeon who specializes in wound management.  He graduated from Hunter College of City University of New York in 2003 with Cum Laude in Biological Sciences, and obtained his Doctorate in Medicine (MD) in 2007 from Boston University School of Medicine. He trained in general surgery and trauma for 3 years at Cooper University Hospital in Camden, NJ.

Since 2010 Dr. Manu has dedicated his career to heal complex wounds as a wound care physician specializing in Subacute Rehabilitation Centers, Skilled Nursing Facilities, and Continuing Care Retirement communities. To ensure continuity of care for his patients Dr.K conducts weekly wound rounds with a certified wound nurse and essential members of the patient care team  to review, educate and implement a treatment plan to obtain optimal healing for his patients.

In addition, Dr. Manu is highly skilled in bedside debridement’s, cauterization, biopsy, incisions and drainage. Eliminating the need for outpatient surgery and reducing the risk of rehospitalizations.

In Dr K’s free time he enjoys traveling ,soccer and spending time with his wife and son.
